Our Care Model

The clinical support that reaches patients where the system doesn't.

Most healthcare is built around appointments. For patients with chronic or complex conditions, the moments that matter most happen in between. Alanté provides continuous clinical support at those moments: after discharge, between visits, and across the transitions where care tends to break.

Built around the patient, not the appointment.

Most care models are built around episodes: a hospitalization, a procedure, a follow-up. We designed ours around the patient, to manage what happens continuously, not just when problems show up.

For patients, that means someone watching over them, ready to act before a bad day becomes an emergency. For the organizations behind them, it means clinical capacity that operates as part of their own team, under their own identity, without the overhead of building it internally.

The result is care that feels constant to the patient and performs measurably for the organization.

Eight areas of clinical support.

Alanté delivers a coordinated set of clinical programs designed to improve outcomes, close care gaps, and support high-risk patients and their families across the care continuum.

Active condition follow-up

Many conditions appear between visits. Others get lost in the handoffs between one care setting and the next. We identify active conditions, create a care plan for each, and follow up on them, so care plans reflect the patient's real state and not the last visit's snapshot.

Preventive care

Preventive services are among the most effective tools in healthcare and among the most consistently missed. We work to keep wellness visits, screenings, and care gap closure on schedule, building the foundation for better long-term outcomes.

Chronic condition management

Patients with multiple chronic conditions require more than periodic check-ins. We provide structured, longitudinal support that keeps patients engaged with their care plans, monitors changes in condition, and intervenes before small problems become costly ones.

Medication management

Medication errors and non-adherence are among the leading causes of avoidable hospitalizations. We manage medication reconciliation, adherence support, and coordination across prescribing providers to keep patients safe and on track.

Remote oversight

Health changes don't wait for appointments. We monitor patients between visits using remote health data, detecting early warning signs and enabling timely intervention before conditions escalate to an emergency.

Care transitions

The period immediately after discharge is when patients are most at risk and least supported. We manage transitions between care settings: from hospital to home, from skilled nursing to primary care, across the handoffs where continuity breaks and complications begin.

Access and support services

Transportation, nutrition, housing instability, and insurance barriers prevent patients from following through on their care, regardless of how good the clinical plan is. We identify and address those barriers so clinical interventions actually work.

Family and caregiver support

Chronic illness affects the whole family, not just the patient. We include caregivers and family members in the care coordination process, so they have the information, support, and resources to help their loved ones navigate the healthcare system.

Five steps. One continuous cycle.

Every patient we support moves through the same process, built to be continuous, not episodic.

01
Identify

We identify patients who would benefit from additional support, prioritizing those at highest risk of complications, avoidable hospitalizations, or care plan failure. This includes patients with multiple chronic conditions, recent discharges, and those with barriers to care that put them at elevated risk.

02
Connect

We reach out personally, build trust, and establish the ongoing relationship that makes continuous care possible. For patients, this means someone who knows their situation and is genuinely present. For organizations, it means a clinical extension that operates under their identity from day one.

03
Coordinate

We align care across the providers, settings, and transitions involved in a patient's journey: managing follow-ups, referrals, medication reconciliation, and post-discharge handoffs so follow-through holds between appointments and care settings.

04
Monitor

We monitor patient health continuously using remote data and regular clinical check-ins, detecting early warning signs and activating support before conditions escalate. Not reactive. Proactive, and watching.

05
Improve

We close care gaps, adjust care plans as conditions evolve, and report measurable outcomes back to the organizations we support. Continuous care becomes documented, actionable results.

Integrated into what is already there.

Four dimensions define how we work inside what you've already built.

Four dimensions: Clinical Continuity, Invisible Integration, Technology as Infrastructure, Measurable Outcomes

Clinical continuity

Care for complex patients requires someone watching continuously, across the providers, settings, and transitions involved. We exist to provide that continuity.

Invisible integration

We operate under your identity and work through your existing workflows, with clinical records your team can access. The capacity arrives without the complexity.

Technology as infrastructure

A proprietary platform maintains a current, real-time record of each patient's health history across the providers involved. It's what makes continuity possible at scale.

Measurable outcomes

Partnerships produce documented results: fewer hospitalizations, higher quality scores, better care experiences, and stronger performance across the metrics that matter most.

When care is coordinated and continuous, the people receiving it stay healthier, and the organizations responsible for them perform better. That is the outcome we are built to deliver.

Let's talk about how this works for you.
